Please follow the procedure below so you can submit your sample submission forms online, hassle-free!
STEP 1: Download sample submission forms. Hover over the downloadable forms and choose sample submission form or click the button below.
STEP 2: After downloading the sample submission forms for the analysis you need, kindly print it, fill it up completely, and scan it (save as .pdf). Make sure that all forms were duly signed. Upload to the Submit the Sample Submission section in the Submission Bin tab. Please wait for our confirmation email before sending your samples (via courier) to our laboratory. Because of the global pandemic, we are currently not accepting the samples personally. Although if it is really needed, please set up an appointment first. Your safety and the safety of our personnel are our priority.
*If you're a student, please ask for a recommendation letter from your adviser (with signature from both parties) . Scan the document (save as .pdf). Upload them to the Submit the Sample Submission section in the Submission Bin tab.
STEP 3: Please wait for the confirmatory email of your appointment date and time before going to the UPLB Nanotech Lab.
STEP 4: We will email you the complete address where you can send your samples after we confirmed everything at our end.
